Does this bar come with a foam rubber gasket

Model LFCC22426/01 refrigerator. Replaced part number PS 3535430 pipe holder because it was broken and leaking. Two weeks after replacing it, it started to leak again. Notice that the foam pad is not worn evenly and the part that goes into the hole is wet. Is the foam pad and the pad that goes into the hole one part? It seems that is the problem. Is there a PN or is there something else to repair it. Also the screws that hold the pipe holder cannot be tightened enough.

According to our research, the foam pad and the pad that goes into the hole are parts of the water tube that comes with the water inlet valve, part number PS12706531. You might need to replace it to fix the issue. The screws not tightening indicates the mounting holes are likely stripped. The pipe holder only supports the tube and does not seal water, so tightening it further will not stop the leak. Using slightly longer screws or anchors can secure the holder, but the leak is resolved by properly seating or replacing the water tube. We hope this information helps!
