Model numbers can be made up of numbers (1005400, for example) or a combination of letters and numbers (LAT1000AAE). The model number will most likely appear on either a paper sticker or a metal plate. Removed the broken shaft by beaking it. Cleaned the metal shaft and coated it with light lubricant. Drove the new shaft on with a rubber mallet.
Would have been helpful to know if there is an easier way to remove the sleeve other than breaking it apart. In this case the sleeve was in bad shape and there was no problem with
